In Shawnee, our regular and walk-in clinics are located on South Gordon Cooper Dr. on the AST campus behind (east of) the new Thunderbird Casino.
We currently see all Native Americans, Soonercare members, and insured AST employees. Additionally, all previously established patients of Dr. "Pat" Chainakul can be seen here.

Our extended-hours clinic is located inside our Little Axe Health Center beside our Specialty Clinic, and located left of the Little Axe Pharmacy.
We currently see all Native Americans, and no appointment is required.
This is an acute care facility that will have the ability to treat non-life threatening illnesses and injuries on a walk-in basis. It has 4 private exam rooms including a pediatric room. Care provided for will include common illnesses, sprains, strains, flu, allergies, and infections. However, if you have a life threatening injury, onset of a heart attack or stroke please call or immediately go to the hospital/emergency room nearest you.
RELOCATION NOTICE:
As of March 7, 2024, PlusCare has relocated to inside the Little Axe Health Center. It is located beside our specialty clinic and is to the left of Little Axe Pharmacy.

Our new state-of-the-art facility is located off Oklahoma State Highway 9 on Little Axe Dr. just east of Lake Thunderbird, across the highway from the Thunderbird Casino, tribal store, and old Little Axe Clinic. This is accessible by taking 156th St. north from Highway 9 and then Little Axe Dr. east (right) to the clinic. Little Axe Dr. does not connect to Highway 9.

Our Vision Clinic is located across Highway 9 from the Little Axe Health Center near Thunderbird Casino in the PlusCare Clinic building. Services include comprehensive eye examinations, medical eye care, precise eyeglass prescriptions and fittings, contact lens fittings, full optical shop, and more.
