See a patient benefit advocate at Little Axe Health Center or Shawnee Clinic for enrollment assistance to determine if you or your family qualify for state SoonerCare/Medicaid-CHIP benefits. For elders 65+ year of age, if you qualify for medicare but do not take medicare coverage, you will need to apply for the Native American exemption to avoid the shared responsibility payment. Please note: descendants do not qualify for Contract Health Services, however still need to maintain an annual SoonerCare enrollment to receive basic clinic healthcare services.
