Patient Registration

Patient Registration provides new and potential patients to the ASTHS with the necessary documentation and paperwork to establish a chart. Applicants will need to provide: State driver’s license or State I.D, tribal enrollment card or CDIB, Social Security cards and if any, insurance card (Medicaid, Medicare, and private insurance.) Please beware that all documentation must be completed in order to establish a chart and schedule an appointment.
